Notice for current/former clients of Residential Treatment Facility program

This notice is for current and former clients of Parsons Child & Family Center’s Residential Treatment Facility (RTF).

On October 29 or 30, 2016, a Parsons laptop computer containing information about clients of the RTF was stolen. A police report was filed, and in addition to the police investigation we also conducted an internal investigation. At this time, the laptop has not yet been recovered. Because the laptop contained private health information about clients associated with the RTF between 1999 and 2015, it is important that those who may be affected know the following:

The types of unsecured protected health information which may be involved in this breach are: full name, date of birth, diagnosis medications, lab results, diet restrictions, height/weight and food allergies for RTF clients.

To the best of our knowledge there was no social security number or credit information involved in the breach.

Most importantly, it is our job to protect the children in our care, and we take responsibility for this theft. We extend our deepest apologies for any inconvenience or stress it may cause. As part of our investigation Northern Rivers Family of Services, the parent company of Parsons, has taken new security steps to prevent a theft like this from leading to a breach of information in the future.
